    State the exact blocked action

    Name the target, intended change, affected system or person, and why the agent cannot continue autonomously.

  2. 02

    Present minimum evidence

    Include the relevant source, observation time, uncertainty, alternatives considered, and potential consequence without exposing unnecessary sensitive data.

  3. 03

    Ask for one concrete decision

    Approval should cover a specific action and scope. Avoid broad questions that can be misread as standing permission.

  4. 04

    Define expiry and continuation

    Record how long the approval remains valid and which unrelated read-only or reversible work can continue while waiting.
